ZoomCare Management Inc. and its affiliates (collectively, “ZoomCare”) are committed to protecting your privacy through our compliance with this policy. This policy describes:

• Our practices for collecting, using, maintaining, protecting, and disclosing your information.

This policy applies only to information we collect in the Website, in email, text, and other electronic communications sent through or in connection with the Website.

This policy DOES NOT apply to information that:

• We collect offline or on any other apps or websites not described in this policy.
• You provide to or is collected by any third party.

Further, this policy does not apply to protected health information. Information that is protected health information may be subject to additional protections, and you may have additional rights regarding such information, pursuant to the federal Health Insurance Portability and Accountability Act of 1996 and its implementing regulations (“HIPAA”), and similar state laws, as applicable.  Please refer to ZoomCare’s Notice of Privacy Policy for more information regarding your protected health information.

Children Under the Age of 13

The Website is not intended for children under 13 years of age, and we do not knowingly collect personal information from children under 13. If we learn we have collected or received personal information from a child under 13 without verification of parental consent, we will delete that information. If you believe we might have any information from or about a child under 13, please contact us at compliance@zoomcare.com.

Authorized Representatives

An authorized representative is an individual who has the authority to create and manage a ZoomCare account on the Website on behalf of a minor child or other individual over whom the authorized individual has legal authority.  An authorized representative may include a parent, guardian, or other personal representative.

A personal representative may authorize and/or have access to ZoomCare account for a dependent child.  The dependent’s ZoomCare account will be linked to the authorized representative’s account until the earlier of the date: (i) the dependent child reaches the age of 13, or (ii) ZoomCare is notified by the dependent’s authorized representative or a court of law or agency with appropriate authority that the dependent has been emancipated, attained legal custody of his or her own health information, or that a different authorized representative has been named.  Each authorized representative will have the same rights to access and communicate through the dependent’s ZoomCare account.  Each authorized representative agrees that if he or she becomes aware of any change in any authorized representative’s relationship to the dependent that may result in the authorized representative no longer qualifying as an authorized representative, the authorized representative will advise ZoomCare of the same.

Information We Collect and How We Collect It

We collect information from and about users of our Website:

• Directly from you when you provide it to us.
• Automatically when you use the Website.

When you download, register with, or use or register through the Website, we may ask you provide information by which you may be personally identified, such as name, postal address, email address, telephone number, and information related to your health care (“personal information”).

This information includes:

• Information that you provide by filling in forms in the Website.
• Records and copies of your correspondence (including email addresses and phone numbers and messages sent via ZoomCare’s chat features), if you contact us.

Automatic Information Collection and Tracking  

When you download, access, and use the Website, the Website may use technology to automatically collect:

• Usage Details. We may automatically collect certain details of your access to and use of the Website, including traffic data, location data, logs, and other communication data and the resources that you access and use on or through the Website.
• Device Information. We may collect information about your mobile device and internet connection, including the device’s unique device identifier, IP address, operating system, browser type, mobile network information, and the device’s telephone number.  However, we do not link your IP address to your personal information unless you have logged into your ZoomCare account.
• Location Information. The Website may collect time information about the location of your device.

The technologies we use for automatic information collection may include:

• Cookies (or mobile cookies). A cookie is a small file placed on your smartphone. It may be possible to refuse to accept mobile cookies by activating the appropriate setting on your smartphone. However, if you select this setting you may be unable to access certain parts of our Website or App.
• Flash Cookies. Certain features of our Website may use local stored objects (or Flash cookies) to collect and store information about your preferences and navigation to, from, and on our Website. Flash cookies are not managed by the same browser settings as are used for browser cookies.
• Web Beacons. Pages of the Website, the App and our emails may contain small electronic files known as web beacons (also referred to as clear gifs, pixel tags, and single-pixel gifs) that permit ZoomCare, for example, to count users who have visited those pages or opened an email and for other related statistics (for example, recording the popularity of certain content and verifying system and server integrity).

Third-Party Information Collection

When you use the Website or their content, you may be directed to our third-party payment processor to handle payment transactions with ZoomCare.  Any payment transactions will be subject to such third-party payment processors’ privacy policies and terms and conditions.

We use information that we collect about you or that you provide to us, including any personal information, to:

• Provide you with the Website and their contents, and any other information, products or services that you request from us.
• Develop, provide, and improve our services, conduct internal reviews of our services, and protect the security and integrity of our services.

We may also use your information to contact you about our own and third parties’ goods and services that may be of interest to you. We may use the information we collect to display advertisements to our advertisers' target audiences. Even though we do not disclose your personal information for these purposes without your consent, if you click on or otherwise interact with an advertisement, the advertiser may assume that you meet its target criteria.

We may disclose aggregated information about our users, and information that does not identify any individual or device, without restriction.

• With third parties who conduct marketing studies and data analytics. These third parties may combine your information with the information of other consumers for purposes of conducting these studies and/or analytics. For example, we use certain Google products to track and report activity within the Services. If you wish to opt out of having your data used by Google Analytics, you may download and install on all your browsers the Google Analytics Opt-out Browser Add-on developed by Google for this purpose, available at https://tools.google.com/dlpage/gaoptout/.

Your Choices About Our Collection, Use, and Disclosure of Your Information

We strive to provide you with choices regarding the personal information you provide to us. This section describes mechanisms we provide for you to control certain uses and disclosures of your information.

• Tracking Technologies. You can set your browser to refuse all or some browser cookies, or to alert you when cookies are being sent. If you disable or refuse cookies or block the use of other tracking technologies, some parts of the Website may then be inaccessible or not function properly.
• Location Information. You can choose whether or not to allow the Website to collect and use real-time information about your device’s location through the device’s privacy settings. If you block the use of location information, some parts of the Website may become inaccessible or not function properly.

We do not control third parties’ collection or use of your information to serve interest-based advertising. However these third parties may provide you with ways to choose not to have your information collected or used in this way. You can opt out of receiving targeted ads from members of the Network Advertising Initiative (“NAI”) on the NAI’s website.

Your browser or mobile device may allow you to adjust your settings so that “do not track” requests are sent to the websites you visit.  ZoomCare will not track user activity once “do no track” signals have been activated.

We may update our privacy policy from time to time. If we make material changes to how we treat our users’ personal information, we will post the new privacy policy on this page.
