Billing help

For claim processing, you can fax our billing department directly at 503-941-3778. If a facility or insurance provider needs to send or discuss claims, our PO Box is 23577, Tigard, OR 97281.

Medical records

If you're a patient, you can view your records and test results online in your ZoomCare patient portal. If you're a medical facility or law office, email your requests to health@zoomcare.com or fax your request to 971-256-0049.

If you're referring a patient to our facility, please email health@zoomcare.com or fax the referral to 866-859-8195.

Having trouble with the ZoomCare website or app? We can help with that! Please contact us at 844-966-6777 or health@zoomcare.com to get your technical issues resolved. Questions about your online account? Sign into your ZoomCare patient portal to submit a technical request.
